EMPLOYMENT VERIFICATION POLICY.

Applies to: EmergencHealth PLLC, EmergenceHealth Management, Essential Anesthesia Management Group, and EmergenceHealth Services, Youngs Professional Services PLLC and all subsidiaries and affiliates thereof
Purpose
To ensure consistency, accuracy, and confidentiality when responding to employment verification requests for current and former team members and clinicians.
Policy Overview
EmergencHealth PLLC, EmergenceHealth Management, Essential Anesthesia Management, and EmergenceHealth Services, YPS, PLLC and all subsidiaries and affiliates thereof (collectively, “the Company”) provides limited information in response to employment verification requests.
In accordance with Company policy and privacy best practices, we do not disclose reasons for separation or other employment details beyond confirming dates of service and position held.
Verification Requests

For Clinicians (CRNAs, Anesthesiologists, etc.):
Employment verification requests should be directed to the Credentialing Department at
credentialing@essentialamg.com -
For Non-Clinical Employees (Corporate and Administrative):
Employment verification requests should be directed to the Human Resources Department at
Onboarding onboarding@essentialamg.com
Information We Provide
We will confirm only the following details:

Full name
-
Dates of employment or service
-
Last position or title held
Information We Do Not Provide
The Company will not disclose:

Reasons for separation or termination

Rehire eligibility

Salary or compensation information (unless authorized in writing by the individual)

Performance information, character references, or opinions
If a verification form includes questions beyond the permitted information, those sections will be marked:
“Per Company policy, only dates of employment and position held are confirmed. No other details are provided.”
If a Verification of Employment (VOE) form requires a “Yes” or “No” response for any question that we do not answer such as “Eligible for Rehire” in order to advance or complete the submission, please do not answer the question directly.
Instead, contact the requesting party and inform them of the following:
“Per company policy, only dates of employment and position held are confirmed. No other details are provided. Therefore, we are unable to complete the form as written.”
This ensures compliance with company policy and limits disclosure to verified, factual employment information only.
